Join a new and innovative community based integrated falls prevention team which will support people who have fallen or are at high risk of falling in Chatham-Kent county. Work collaboratively with other community health care partners to help people rapidly regain strength and their functional independence, learn strategies to prevent future falls and increase the safety of their environment.

Working alongside Community Health Professionals, often completing joint visits to respond to situations where someone has suffered a fall. It is a unique opportunity to deliver more integrated care, and truly intervene on the silent epidemic of falls. competitive, salaried position with regular work hours. What you need to be successful 

  • You are registered as a member in good standing with the College of Occupational Therapists of Ontario
  • Your focus is to embrace person and family-centered care
  • You are passionate, empathetic, use good judgement and apply critical thinking when making client decisions
  • You are known for your excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently
  • You value the role a therapist plays in keeping clients at home when that is of personal importance
  • You have the ability to travel within the Chatham/Kent community as part of the CBI Home Health team
