Responsibilities:
Reporting to the Manager, Transitional/Palliative/Complex Care Unit, Seniors Care & Discharge Planning, the Discharge Planner will work within a fast paced environment as a collaborative team member supporting and enabling patients and families to make timely transitions through the hospital system utilizing a home first philosophy. They will work collaboratively with inter professional teams, along with community partners such as Home and Community Care Support Services, to ensure a coordinated, efficient and integrated approach to care delivery and transition back to the community. Being one of the key points of contact for discharge planning to formulate, facilitate and coordinate discharge plans focused on the needs and goals of the patient and their family system, the Discharge Planner will also be responsible for:
- Using creative problem solving and critical thinking to facilitate all discharge planning, including complex discharge planning situations
- Collaboratively creating the discharge plans with the team, the patient and their family and providing support, education and guidance through the implementation of the discharge plan
- Facilitating a timely transition to the community using a home first philosophy; addressing all barriers using most appropriate resources in partnership with internal and external partners
- Supporting an environment that promotes continuous quality improvement and staff engagement to achieve established clinical benchmarks and outcomes
Qualifications:
- Licensed member of a relevant health profession
- Current registration and membership in good standing with respective regulated college
- 2-5 years clinical experience in an acute care hospital setting
- Experience working within and supporting a Home First Philosophy
- Current knowledge of community resources and initiative to proactively search for new resources
- Experience with legislation that governs Public Hospitals (Public Hospitals Act, Substitute Decisions Act, Consent to Treatment Act, etc.)
- Demonstrated knowledge base of resources in the public and private sector, and medical and clinical language and procedures.
- Knowledge of how to effectively serve an increasingly aging and diverse population with complex medical, functional, cognitive and psychosocial needs
- Comprehensive knowledge of the impact of social determinants of health on discharge planning, hospital LOS, Patient Flow and hospital system capacity
- Excellent communication, collaboration, organizational and problem solving skills
- Ability to work independently as well as with an interdisciplinary team
- Demonstrated ability to set priorities
- Skilled working with a broad range of client groups and their significant supports
- Satisfactory employment and attendance record
