Designed for occupational therapists and other healthcare professionals across settings, this session focuses on simplifying discharge planning while addressing the real-world complexities clinicians face every day.

Presenters:
Stacey Baboulas, OT Reg. (Ont.), Hon. BSc. (Kin), MSc.O.T., Stacey Baboulas & Associates, is a Registered Occupational Therapist with the College of Occupational Therapists of Ontario (COTO), holding a Master of Science in Occupational Therapy from the University of Toronto and a Bachelor of Science in Kinesiology from York University. She is a Certified Canadian Life Care Planner (CCLCP), a Certified Impairment and Disability Rater (4th Edition), and a certified Progressive Goal Attainment Program (PGAP) clinician. She has also completed advanced brain injury coursework through the Ontario Brain Injury Association in partnership with Brock University. With over 15 years of clinical and medico-legal experience, Stacey works with individuals recovering from motor vehicle accidents, workplace injuries, and other disability-related conditions. Her areas of focus include discharge planning, accident benefit assessments, attendant care evaluations, catastrophic impairment assessment, and developing individualized treatment programs. Stacey also prepares Future Cost of Care and Loss of Services Reports. She has delivered presentations on complex discharge planning and the role of occupational therapy in home and community assessments, and also serves as a clinical supervisor for Occupational Therapy students at the University of Toronto, supporting the development of future practitioners. Stacey is an active member of the Canadian Association of Occupational Therapists, Ontario Society of Occupational Therapists, Brain Injury Society of Toronto, and the International Association of Rehabilitation Professionals.
Leigh Craney, R.R.P, Leigh Craney & Associates, holds a Bachelor of Science degree from Brock University and has been working as a Community Case Manager since 2003. Over the course of her career, she has developed extensive expertise supporting individuals with complex needs, particularly in the areas of brain injury and spinal cord injury rehabilitation. Leigh has successfully completed advanced educational training in these specialties through the Ontario Brain Injury Association (OBIA), as well as through Brock University, McMaster University, and the University of Calgary. This training has strengthened her ability to provide informed, client-centered care and to collaborate effectively within interdisciplinary teams. Leigh is an Accredited Registered Rehabilitation Professional (R.R.P.) through the Vocational Rehabilitation Association of Canada and an active member of the Ontario Rehab Alliance. Her work is grounded in a commitment to advocacy, evidence-based practice, and helping clients achieve meaningful independence and quality of life.
