My Transitional Care Plan During the COVID-19 Pandemic Tool

This webinar will introduce the My Transitional Care Plan during the COVID-19 Pandemic (MTCP-C19) tool. The webinar will provide an overview of the tool, how and why it was developed and how its use has impacted transitions during COVID-19. The presenters will also speak to the important role occupational therapists play during a resident’s transition in to or out of a setting, and the ways in which occupational therapists can implement the MTCP-C19 tool to help facilitate a successful transition.

Session Topics:

  • Reflect on the unique challenges when supporting individuals to transition across care settings during the pandemic and ways these can take place more seamlessly in the future by fostering better communication and information sharing around critical information normally lost in transitions.
  • Identify the critical elements needed to support transitions into long-term care and retirement homes for older adults living with dementia, complex mental health, substance use, and other neurological conditions.
  • Discuss the important role that occupational therapists (OTs) play in facilitating transitions and equip them with a new tool they may consider incorporating into their practice.
     

About our Presenters:

Jillian McConnell
is a Knowledge Broker and Knowledge Mobilization Lead for the brainXchange where she is responsible for both the national and provincial elements of the network.

Katelynn Aelick is the Project Coordinator for the Behavioural Supports Ontario (BSO) Provincial Coordinating Office at the North Bay Regional Health Centre. As part of her role, Katelynn leads initiatives focused on measuring the impact of BSO and surfacing emerging and promising practices on the topics of capacity building, transitions and sexual expression.

Kim Schryburt-Brown is an Occupational Therapist who has worked in community Seniors Mental Health at Providence Care for the past 17 years.
